Mission

The Comanche Nation Diabetes Program’s mission is to promote a healthy, self-care lifestyle by providing education and guidance in the areas of prevention and control of diabetes and physical and nutritional health, which will benefit Comanche Tribal members.

Services

The Comanche Nation Diabetes Program Provides:

• Blood sugar meters
• Monthly test strips & lancets
• Diabetic footwear
• Annual foot exam/prescription
• Medication assistance
• Walkers, rollator, wheelchairs, and knee scooters (limited to new amputees)

Eligibility Requirements

• Comanche Tribal ID
• Diagnosis of Type 1 or 2 Diabetes
• Verification of Residence
• Prescription
