Mission
The Comanche Community Health Representative (CHR) Program provides community-based health support to Native American individuals and families. CHRs act as liaisons between the community and healthcare providers by offering advocacy, health education, care coordination, transportation assistance, and home visits.
Our program focuses on improving access to medical services, supporting health management, and empowering community members to make informed health decisions. Through culturally respectful outreach and personalized assistance, the CHR Program works to enhance overall wellness and strengthen the health of our tribal communities.
Service Area
Comanche, Cotton, Tillman, parts of Caddo, Jackson, Jefferson, and Stephens Counties.
Services
Comanche Nation CHR is contracted with Indian Health Services for funding to use these resources and services for all tribally enrolled Native Americans within program’s service area.
- Medical transportation to and from routine doctor appointments
- Home health visits for homebound individuals
- First-aid standby for powwows or special events
- Rabies vaccination clinics
- Medication pick-up and drop-off from IHS Pharmacy
Additional Assistance (Comanche Enrolled Only)
- Medical Appointment Fuel Assistance for individuals that reside outside program’s service area
