Caregivers ProgramWe provide assistance to all eligible Native Americans sixty (60) years of age and older and grandparents sixty (60) years and older raising grandchildren who are eighteen (18) years and younger. They must live in our geographic service areas of Comanche, Cotton, Tillman counties and portions of Jackson, Jefferson, Stephens and Grady Counties and including towns and cities of Cyril and Apache, Ok and Comanches living in other parts of the state need to apply to the program and need doctor’s order’s with diagnosis and condition of client. Need a CDIB for proof of tribal membership. If you are a caregiver who is sixty (60) years or older or if you are a grandparent sixty (60) years or older raising your grandchildren who is eighteen (18) years or younger, you may be eligible for assistance with the elderly requiring two (2) activities of daily living. AssistanceAnd chronic conditions leading to disability : Heart disease, Stroke, Diabetes, Pulmonary disease. Affecting functioning ability: Arthritis, Vision and Hearing loss , Osteoporosis, Impairment, Hypertension, and other illness and diseases. |
