How to Apply for Assistance

Some items you will need to bring with you when applying for assistance:

1. Comanche C.D.I.B

2. Prescription

3. There is no age limit or income requirement.

4. Must complete an application when applying for assistance.

It is the patients responsibility for getting and submitting all the necessary documents that is needed when applying for assistance.

This assistance is not meant to replace any existing resources that the client may have access to, such as Medicare, Medicaid, Title IX, Private Insurance, DHS, VA, or any other resource.
