Community Health Representative (CHR) Program

In accordance with the provision of 42 code of Federal Regulations, Prot 36116, the Comanche Nation will administer a Community Health Representative, Emergency Medical Service Program funded under the Indian Health Service.

The purpose of the Community Health Representative and Emergency Medical Service Program is to be an advocate and act as a facilitator for individuals and families to gain access to comprehensive health care services.

Goal: To provide for a continuum of service to the population through Health Education, Case Finding referral, follow up and provide support services designed to enhance the quality of life for the people they serve.

Objective: To target population is all the Native Americans residing within the Comanche CHR / EMS service area in Comanche, Cotton, Tillman, and portions of Kiowa, Stephens, Jackson, and Jefferson counties in Southwest Oklahoma.

Services Provided:
Health Education - in home, while transporting.
Screenings - health fairs, workshops, office.
Case Management - make appointments arrange transportation, patient advocate.
Monitor - bloody pressure / blood sugar / medication check.
Patient Care - change dressing, medication pickup and delivery.
Transportation - local, Oklahoma City, Norman, Anadarko.
Translate - what medical doctor has said, medication.
Environmental Services - house in need of repair, driveways, ramps, water, rabies clinic, etc.
Training - basic CHR course/refresher course, mandatory requirement for all CHR / EMS personnel.

Emergency Medical Service Program (EMS)

Goal: to provide the Comanche Nation and all Native American's living in their service area with Emergency Medical Service. A training component to provide CPR / First Aid Training to community members. A medical control procedure in conjunction with the IHS and ongoing reporting.

Services Provided:
CPR / First Aid Classes
Monthly AED checks @ all outlying tribal facilities
Maintain current updates of EMS / CHR skills
30% of work schedule includes working in CHR service area, i.e. transporting, home visits, medication pickup/delivery.
Attend meetings with FEMA
Assists with car seat.
